Income Tax forms at the library

The library has NO income tax appointments left.  We can add people to a waiting list, but there is NO guarantee that those on a waiting list will get an appointment.  The library maintains a list of other organizations that offer tax assistance for those who are in need of assistance.

The Woburn Public Library has a limited supply of Federal tax forms available this year.  Copies of the IRS forms 1040, 1040A and 1040EZ along with select schedules are on display in the Reference area of the library.  Additional forms can be found on the IRS website http://www.irs.gov and can be printed from library computers for a fee of $0.20 per page.

The MA Department of Revenue (DOR) has sent a limited supply of tax forms and instructions to libraries.  They are on display in the Reference area of the library with the Federal tax forms.  Additional MA tax forms can be printed from the Department of Revenue website http://www.mass.gov/dor for a fee of $0.20 per page or can be obtained by calling the DOR to request a mailed copy at 617-887-6367, or toll free in MA at 1-800-392-6089.

Additional tax assistance can be found at the local IRS office at 1 Montvale Ave in Stoneham.  The hours are Monday–Friday 9am-1pm and 2pm-3:30pm.
